Dropped Objects Prevention Plan

Below is the Dropped Objects Prevention Plan that the API shop has created.
Laminated copies will be placed in all the trucks. If you would like an additional copy please see the Wells Supervisor or the Wells Admin.


DROPPED OBJECTS PREVENTION PLAN

Boom Truck / Crane Inspection

1. Ensure crane operator has current certification.
2. Ensure riggers are trained and competent.
3. Ensure yearly and monthly crane inspections are current.
4. Conduct daily visual inspection. (Ensure safety devices are installed, tested & fully operational)
5. Conduct practice pick without load.
6. Know the weight of load, angle & radius of boom, and do lift calculations.
7. Secure tag lines.
8. Full equipment inspection required after “shock loading”.

Below the Hook Requirements

1. Ensure annual inspection markings are visible on all lifting equipment. (2010 = Black)
2. Ensure load ratings on all synthetic fabric, wire rope & steel alloy slings are legible.
3. Visually inspect slings and shackles for wear/cracks prior to use.
4. Ensure slings & shackles are appropriately rated for desired application.

Weather

1. Verify wind speed. (Wind speeds over 20 mph requires an assessment/variance).
2. Verify temperature. (Temperatures below -35 F requires an assessment/variance).
3. Apply proper de-rating when applicable.

Hand Tools

1. Inspect scaffolding for kick plates/toe boards.
2. Limit the amount of tools present on tree platform.
a. Utilize tool bags when multiple tools/equipment are required.
3. Secure lanyards to Parmalee wrench, crescent wrenches, pipe wrenches, etc.
4. Clear all employees from underneath working areas before starting, and ensure “level to level communication” to prevent others from walking underneath during the job.

Pre-Job

1. Dropped objects prevention shall be discussed in pre-job meetings and noted on SETA cards to raise awareness.
